Your First Visit

WELCOME TO CENTERPOINT ENDODONTICS

Your first visit will include a comprehensive consultation. We’ll take a high-resolution 3D X-ray (CBCT scan), review your health history, and discuss a personalized treatment plan that meets your needs. In many cases, treatment may begin the same day.

Please assist us by providing the following information at the time of your visit:

A state issued ID.
If you have dental insurance, please bring your insurance card.
A parent or guardian must accompany all patients under 18.
A list of your current health conditions and medications.
If you require medication prior to dental procedures such as antibiotic prophylaxis or anxiolytics.
